CAD Emergency Information Form

The Eaton Police Division’s Computer Aided Dispatch System (CAD) provides first responders with critical information when responding to an emergency.  The system has the ability to associate your name, address, or both, with special information or instructions that can greatly assist first responders if called to provide emergency assistance to you or your family. 

If there is any important information or special instructions you would like to provide us with that you feel would assist us if called to help you or your family, we will enter the information into our CAD system. 

All information will be kept strictly confidential and will only be used in the event of an emergency.
